UHS Population Health

The UHS Population Health team supports patients by addressing essential non-clinical needs such as food insecurity, housing, and transportation. Through innovative programs—including an in-house food pantry—and strong partnerships with local organizations, the team helps patients feel safe, secure, and supported. Population Health also works to secure funding for housing needs, ensuring patients receive the resources they need beyond medical care.
